As one colouring book design guide puts it, the difference shows up the first time a child actually colours the thing in.

That is a fixable problem rather than a reason to avoid the tools. The fix: ask for line art on a plain white background, no shading, no filled areas. Then look at the result and ask whether a printer would empty a cartridge on it.

Why does the page size cause trouble?

Because a free AI image generator does not produce paper shapes, and there are two different paper shapes in play.

A4, standard across the UK and Europe, measures two hundred and ten by two hundred and ninety-seven millimetres. US Letter, standard in North America, is eight and a half by eleven inches. Those are different proportions, so a page designed to fill one will be cropped or letterboxed on the other.
